Purpose

The purpose of this privacy policy is to explain the types of information the SPARS website collects and maintains about visitors to the site, how the information is obtained, and how it is used.

Information Automatically Collected and Stored

When you browse through any website, certain personal information about you can be collected. The SPARS website automatically collects and temporarily stores the following information about your visit:

  • The name of the domain you use to access the Internet
  • The date and time of your visit
  • The pages you visited
  • The address of the website you came from before you visited
  • The type of browser you use

This information is used to improve the website and to provide a better user experience for visitors. Unless it is specifically stated otherwise, no additional information will be collected about you. The SPARS server logs collect additional technical information, such as error messages, which are stored as a record of service. This information is never used to identify visitors to the SPARS website. For a complete list of the types of information that the server logs, please send an email to SPARSHelpDesk@mathematica-mpr.com. When inquiries are sent via email, the question and email address are stored so staff can respond electronically; however, that information is not publicly identified unless required by statute. The SPARS website does not collect personally identifiable information (PII) about you unless you choose to provide that information. Any PII you chose to provide is protected by security controls consistent with the HHS Information Security and Privacy Policy and National Institutes of Standards and Technology and Office of Management and Budget guidance.

Personally Provided Information

You are not required to provide personal information to visit the SPARS website. Specific information, however, is required for you to fully participate in the SPARS Training Site. The personal information that is collected to facilitate the account setup is as follows:

  • Email address
  • Organization
  • Title
  • Address line 1
  • Address line 2
  • City
  • State
  • ZIP code
  • Office phone
  • Home phone
  • Cell phone
  • Fax

If you choose to provide the personal information requested, the information will only be maintained as long as necessary to fulfill the stated purpose for the data collected. The information will be regularly archived. The information is protected by the Privacy Act, which restricts the use of such information based on certain disclosures.
